New or on going medical conditions - how to get help?

If you  have a new or on going medical condition and you need to see one of our clinicians then complete the online form,  Online form

If you are unable to complete the form then contract the surgery on 01748 876090. Our phone lines will be busy, but you will be asked the same questions as the online form.

You can call into surgery and you will be asked the same questions 

Once completed, the form will be reviewed by the duty clinican who will decide based on your clinical need what the best course of action is .....

This may include, advice, referral to a pharmacist, a routine appointment or an urgent face to face or telephone appointment. 

The surgery will contact you by text or telephone after your form has been reviewed.

Total Triage

The way our patients will access urgent & routine appointments is changing ...........

From Monday 20th October we shall be using a new system called Total Triage.

This will ensure that every patient gets the right care , at the right time, by the right healthcare proffesional.

To help all of our patients we have pulled to together some questions and answers

Routine Nurse and Blood Appointments

You can book your routine nurse appointment and blood test via reception 

Better Access - More routine primary care appointments

More routine primary care appointments have been made available to people living in our area. Patients registered at GP practices in Hambleton and Richmondshire can be offered appointments between 6:30pm and 8pm weekdays and between 8:30am and noon on Saturdays and Sundays. This is not a walk-in service 

When submitting an online form about a new or on going condition and an appointment in these times frames may be better for you, just include it on the form 

Home visits

We prefer to see patients in the surgery whenever possible, as we then have all the facilities available. However, where a patient cannot attend the surgery for medical reasons we will see them at home. If you feel you need a home visit please let us know as soon as possible and a GP/ANP will call you back to assess the situation.

Remember: your doctor can usually see several patients at the surgery in the time that it takes to make a single house call.

You can help us!

Please be on time for your appointment by arriving a few minutes beforehand. If you are more than five minutes late for your appointment, you may not be seen.

Why not arrive 10 minutes early and use the Patient POD ...... it wil measure and record you blood pressure, heart rate, height, weight and BMI to your patient record.

All telephone calls to and from the medical practice are recorded for quality and staff training purposes.
